Pre-Participation Physical Evaluation Form

MHS Athletic Physical Form.pdf

The State issued Pre-Participation Physical Evaluation Form must be completed and signed by the athlete and a parent/guardian (pages 1 & 2). The entire form (4 pages) must be printed and then submitted to the family physician. He/She must complete and sign pages 3 & 4. Return the entire form (pages 1 - 4), with all signatures, to the Athletic Office. Download the State Medical Form. Parent/Guardian complete pages 1 & 2. The family physician completes pages 3 & 4. Return the entire form to the Athletic Office. The MHS school physician will review the physical for final clearance to participate in sports.

Health History Update

To participate on a school-sponsored interscholastic or intramural athletic team or squad, each student whose physical examination was completed more than 90 days prior to the first day of official practice shall provide a health history update questionnaire completed and signed by the student’s parent or guardian. Download the Health History Update form. Parent/Guardian complete the form, upload it back into the registration portal and submit a signed hard copy to the Athletic Office. *Athletes must complete this form for each sport that you play in the same calendar year.
